Fix incompatibilities between XML output and Hudson's cobertura plugin

Several issues with how gcovr generates XML output were identified in  HUDSON-5235. In particular:

  • relative paths to file names should not begin with a leading '/'
  • the report should include a <sources> section to identify paths to the source files.

I think I fixed the first issue by using the patch.

The second issue does not seem to be relevant. We _do_ support a 'sources' directory?

I'm assigning this to John so he can confirm that I've addressed the issues in this thread. (I had some difficulty figure out what the specific fixes where that were required.

bugfix: gcovr was only including a <sources> element in the XML report if the user specified -r (resolves #3869)
